Intake Process

Contact Us

Call or email to request an intake form and have the information emailed to you. We will gather basic information and answer any initial questions.

Submit Intake Information

Return the completed intake form and any additional requested details.

Schedule the Evaluation

Upon receiving the intake information, We will schedule an evaluation with both the caregiver and the child. This visit includes assessment and discussion of your concerns and goals.

Review of Findings

The evaluation will be reviewed with you, and the recommended treatment plan will be discussed.

Begin Occupational Therapy

Once the treatment plan is agreed upon, OT appointments will be scheduled based on your child’s needs and your availability.

FAQ


Indiana allows families to begin occupational therapy without a referral for the first 42 days or 12 visits (whichever comes first). After that point, a referral is required to continue services.
While a referral is not required to start, it is generally considered best practice. We may treat without a physician referral on a case‑by‑case basis when the needs are expected to be straightforward and short‑term.

Evaluations usually last about an hour in the clinic. After the visit, we continue scoring and compiling the results over the subsequent days. Most families return the following week for a report‑of‑findings appointment, where we review the results together and discuss next steps.

We do not offer teletherapy at this time. In‑person therapy is typically more effective for the types of hands‑on, play‑based, and movement‑based interventions we provide, and it is usually the most appropriate option whenever possible.

We primarily work with school‑aged children, typically ages 4 to 21. Older individuals may be seen on a case‑by‑case basis depending on their needs and the appropriateness of our services.

Cancellations must be made at least 24 hours in advance. OT4ME reserves the right to charge a $50 missed‑appointment fee for cancellations with less than 24 hours’ notice. We understand that illness and true emergencies happen, and occasional situations such as a child waking up sick will not incur a fee.

Consistent attendance is important for progress, and we ask families to commit to the schedule they accept. If regular attendance becomes difficult, we’ll talk through options together. Full scheduling expectations are reviewed at the evaluation and included in the intake paperwork.

We accept payment by cash, check, or debit card (including HSA cards). Payment is due at the time of treatment unless other arrangements have been made in advance.

We accept patients when we believe our services are appropriate and likely to help. If a person’s needs fall outside the scope of occupational therapy or would be better addressed by another provider, we will help guide you to the right place.
